No reloads, no refreshes, but every topic submitted by a user is created twice and the most recently created is presented to the user submitting it. Every time somebody posts, I need to go around delete and then purge the one created in excess.
A restart fixes it for a while and I can see it happening again after a few days. Dunno what is causing it and how to find out.

So, while trying to recreate this, I noticed that the duplicate is only happening when the guest types in a 'custom' name for himself. If he leaves the name field blank, the post is created once and all goes fine.
As soon as the guest enters his custom name in the name field, the topic is created twice.
Yes, I have given guests the permissions to create topics.
Should I open an issue at GitHub or posting here should be fine?
@Narender-Singh should be better to post a github issue in order to get it fixed. Make sure you write the commit has your using /version, replication steps etc.
This is already fixed on master, it is backported to 1.x.x now https://github.com/NodeBB/NodeBB/commit/550fa4717d07ebe23a03424c84ab6b0ea724f493
