Email title is redundant, and has an unescaped &

I'm testing out some of the email that get sent out and I have two problems.

1. The forum title is displayed twice
2. The second forum title converts the & to a &

Any thoughts on how to fix this?

Update: this is possibly only on the reset password email. I'm still testing out various emails.

@baris Confirmed it is a bug. I think this was one of the first bugs I found so I wasn't sure if it was a misconfiguration or not.
