A lot of things around moderation, spam, etc could be addressed via common libraries or in requirements for the conformant protocol.
Those things are simply not there and are not considered.
You can see this in the ActivityPub book: you get to build your own bespoke solution to flagging! These are the sorts of things that should be clearly and unambiguously defined for any single operating social network.

I'm glad the feature is there now and now I'm also wondering if there could be an automatic user notification as well - i.e. if no moderator activity for N period (3 months? The period that mods are supposed to notify users anyway if they're closing the instance) that there is an announcement and perhaps a simultaneous post on whatever the contact / mod account on the instance is that has some system statement "there has been no moderation activity for N, users may want to migrate".
