Yeah this would be a good plugin. Right now the gravatar subsystem is very tightly integrated with NodeBB, so it's going to take some effort to untangle it
Open an issue on Github, please. We can more easily track things there.

Thanks should be fixed in https://github.com/designcreateplay/NodeBB/commit/1a9ab9d1d2fc273eb7875a40e7bde6d9dec7ac5c, and yes images need to be accessible on port 80 or 443.
